From the FAA’s Accident/Incident Data System (AIDS), through its Aviation Safety Information Portal, as reported. The NTSB investigates accidents separately, and its record of an accident is the fuller one. The aircraft is matched to the registry by N-number and event date, checked against the reported make and model; the registry’s history has gaps, so older matches can be wrong. Compare the reported make and model.

What happened

The FAA’s narrative, in sentence case. The FAA writes it in capitals and removes private details.

On lift off airc raft contacted dolly and rolled over onto its side. (.4)the pilot reported he was attempting a takeoff from a dolly when the accident occurred. According to the FAA inspector the pilot reported that during initial liftoff, when he applied the collective and raised the helicopter to hover, the rotor RPM bled off and the helicopter settled back on the dolly. He further stated that when he tried to lift off again, the helicopter rolled over. The FAA examined the aircraft after the accident and it was revealed that the skid hooked onto the dolly as the pilot applied power. There was no evidence of mechanical malfunction noted.
