From the FAA’s Accident/Incident Data System (AIDS), through its Aviation Safety Information Portal, as reported. The NTSB investigates accidents separately, and its record of an accident is the fuller one. The aircraft is matched to the registry by N-number and event date, checked against the reported make and model; the registry’s history has gaps, so older matches can be wrong. Compare the reported make and model.

What happened

The FAA’s narrative, in sentence case. The FAA writes it in capitals and removes private details.

(-16) On August 22, 1995, at 1120 CST, an airtractor, N3169U was substantially damaged while taking off from a plowed field near Dalhart, TX. No injuries were sustained by the lone occupant, a commercial pilot. Visual meteorological conditions prevailed for the agricultural flight. The airman medical qualifications were involved because the pilot's medical had expired (even as a 3rd class) on 08/01/95. Further, the pilot presented a medical certificate to the investigating inspector that had the issue date altered. This matter is under investigation.
