From the FAA’s Accident/Incident Data System (AIDS), through its Aviation Safety Information Portal, as reported. The NTSB investigates accidents separately, and its record of an accident is the fuller one. The aircraft is matched to the registry by N-number and event date, checked against the reported make and model; the registry’s history has gaps, so older matches can be wrong. What happened

The FAA’s narrative, in sentence case. The FAA writes it in capitals and removes private details.

Narrative: ccr flight 402 departed manaus, brazil (sbeg) to Miami, Florida (kmia) 1540 UTC. Approximately one hour after take-off, the crew noted a decrease of hydraulic fluid in the utility system. The crew followed the company procedures to isolate the system. Subsequently they experienced a total loss of hydraulic fluid. The crew elected to continue to Miami. When they reached Miami ARTCC airspace, they informed ATC of the problem. The flight received clearance to hold at junior, where they attempted to manually extend the landing gears. The flight engineer was unable to lower the nose gear at this time. He was successful with the main gears. The crew attempted to lower the gear by alternate means. This was unsuccessful. They requested a fly-by at Miami to confirm the status of the landing gear. Tower confirmed that the nose gear was up. They prepared for a nose gear up landing on 9R. After landing, the crew executed an emergency evacuation of the aircraft. The subsequent investigation found a defective hydraulic pressure relief valve. This investigation was unable to determine the reason for the malfunction of the emergency gear extension. The f/o and the f/e will be scheduled for a company pc check on April 2, 1996. The PIC was scheduled for a pc check on March 26, 1996. He failed to appear for the check. privacy data omitted

As the FAA filed it

^PRIVACY DATA OMITTED^
