From the FAA’s Accident/Incident Data System (AIDS), through its Aviation Safety Information Portal, as reported. The NTSB investigates accidents separately, and its record of an accident is the fuller one. The aircraft is matched to the registry by N-number and event date, checked against the reported make and model; the registry’s history has gaps, so older matches can be wrong. Compare the reported make and model.

What happened

The FAA’s narrative, in sentence case. The FAA writes it in capitals and removes private details.

Narrative: during takeoff roll from runway 8L at lima intersection, #2 engine torque loss noted at approximately 50 KTS (indicated airspeed). First officer called abort and captain initiated abort procedures. ATCT was informed of abort and no assistance was requested. As aircraft was clearing runway 8L at delta taxiway, #2 engine fire indication, with associated alerts, began. Aircraft was stopped and engine fire on ground procedures initiated. ATCT was advised, an emergency was declared and emergency equipment was requested. Approximately 30 seconds after both engines were shutdown and the first fire extinguisher was discharged, the second agent was discharged. Approximately 10 seconds following discharge of the second agent, all fire indications ceased. In anticipation of a possible emergency evacuation, the main cabin door was opened following shutdown of both engines, but no evacuation was required. Upon arrival at the aircraft, emergency vehicles confirmed there was no evidence of fire and the aircraft was towed to the gate, where passengers were deplaned and reaccommodated on another flight. Investigation by mahalo air pmi found no. 1 fuel nozzle leaking from o-rin g. the installed o-rings (pre-fix as) are being replaced with new o-rings (prefix st). Mahalo air was using old type o-rings because new type o-rings were not available. They have now received the new type o-rings and are making plans to install them on all engines. No NMAC -- no pilot deviation - incident IWP1396022 is closed.

As the FAA filed it
