From the FAA’s Accident/Incident Data System (AIDS), through its Aviation Safety Information Portal, as reported. The NTSB investigates accidents separately, and its record of an accident is the fuller one. The aircraft is matched to the registry by N-number and event date, checked against the reported make and model; the registry’s history has gaps, so older matches can be wrong. Compare the reported make and model.

What happened

The FAA’s narrative, in sentence case. The FAA writes it in capitals and removes private details.

Narrative: an aerospatiale ATR-42, aircraft serial number 014, registration number N421MQ, operating as American eagle flight 4304, landed at o'Hare airport, Chicago, Illinois at approximately 0800 CDT 5/05/97. As the aircraft was taxiing to the gate by terminal "l", the pilot requested emergency equipment, due to a smoking left main gear. Aircraft was stopped just off the taxiway, passengers were deplaned through the normal passenger entry door at the rear of the aircraft. Emergency equipment responded, no evidence of fire. There was no emergency evacuatuion of this aircraft. The aircraft's #1 main tire (lt. outbd) was blown and this same axle area had damage from the wheel bearing coming apart. The tire had evidence of hydraulic fluid that came from the brake assembly. Maintenance had trouble removing the #1 wheel due to parts being welded to the axle. The inboard bearing (large bearings) were disfigured from heat and friction. Maintenance changed the left main landing gear. Sima maintenance director dan maddy reported the wheel had been installed for 2 months and had 390 cycles (average has been 900), also suspect a bearing failure. Note: it was also reported this is the third reported bearing failure on an outbd axle. The pmi was briefed and a follow up will be done by the sima cmu.
