From the FAA’s Accident/Incident Data System (AIDS), through its Aviation Safety Information Portal, as reported. The NTSB investigates accidents separately, and its record of an accident is the fuller one. The aircraft is matched to the registry by N-number and event date, checked against the reported make and model; the registry’s history has gaps, so older matches can be wrong. Compare the reported make and model.

What happened

The FAA’s narrative, in sentence case. The FAA writes it in capitals and removes private details.

Narrative: at approximately 1400 local on June 28, 1997, a parachute drop was executed from the seattle class b airspace over the community of Renton, Washington. During his descent, the parachutist cut away from his main parachute and deployed his emergency reserve parachute. The parachutist impacted a soft field at a high rate of descent and sustained serious injuries. Subsequent investigation revealed that the reserve parachute had not been inspected/repacked within required time frames. It was also determined that the operation may have required a certificate of authorization issued by sea FSDO and that the aircraft was altered without appropriate documentation.
