From the FAA’s Accident/Incident Data System (AIDS), through its Aviation Safety Information Portal, as reported. The NTSB investigates accidents separately, and its record of an accident is the fuller one. The aircraft is matched to the registry by N-number and event date, checked against the reported make and model; the registry’s history has gaps, so older matches can be wrong. Compare the reported make and model.

What happened

The FAA’s narrative, in sentence case. The FAA writes it in capitals and removes private details.

Narrative: during post flight inspection following emergency procedures training with Mr. Keith yamamoto from airstar helicopters, damage to the tailboom was discovered beneath the main rotor tip path. The training sortie had gone well with emphasis on touch down autorotations. Keith had a tendency to hold the collective full up after touchdown; however, his touchdowns were smooth. privacy data omitted Touchdown was preceded by a tail skid strike, then the heels of the skids and as the nose rotated down following touchdown he pulled aft on the cyclic with the rotor RPM low and the collective full up. No indication was felt in the cockpit of the strike. We then completed the directional control failure training with several patterns and touchdowns, finishing his training. All times are approximate. Ratings continued: pilot, airplane multiengine land, N-265, commercial privileges, airplane single engine land, glider aero tow only, rotorcraft-helicopter, instrument helicopter.

As the FAA filed it

^PRIVACY DATA OMITTED^ TOUCHDOWN WAS PRECEDED BY A TAIL SKID STRIKE, THEN THE HEELS OF THE SKIDS AND AS THE NOSE ROTATED DOWN FOLLOWING TOUCHDOWN HE PULLED AFT ON THE CYCLIC WITH THE ROTOR RPM LOW AND THE COLLECTIVE FULL UP.
