From the FAA’s Accident/Incident Data System (AIDS), through its Aviation Safety Information Portal, as reported. The NTSB investigates accidents separately, and its record of an accident is the fuller one. The aircraft is matched to the registry by N-number and event date, checked against the reported make and model; the registry’s history has gaps, so older matches can be wrong. Compare the reported make and model.

What happened

The FAA’s narrative, in sentence case. The FAA writes it in capitals and removes private details.

(-23) This incident was brought to my attention 9/20/99. After checking all resources I determined that this office never received the first 8020-11 that would have arrived via fax around January 4, 1998. On 9/20/99 I requested ARTCC quality assurance send me a new fax on this incident. September 21, 1999 I received the fax, and it wasn't until today 10/04/1999 that this incident was once again brought to my attention. There is insufficient information 19 months after the incident to continue with any action. I have determined that this incident happened too long ago and has thus become stale.
