From the FAA’s Accident/Incident Data System (AIDS), through its Aviation Safety Information Portal, as reported. The NTSB investigates accidents separately, and its record of an accident is the fuller one. The aircraft is matched to the registry by N-number and event date, checked against the reported make and model; the registry’s history has gaps, so older matches can be wrong. Compare the reported make and model.

What happened

The FAA’s narrative, in sentence case. The FAA writes it in capitals and removes private details.

Engine quit on liftoff. Autorotated yawing on touchdown. Skid collapsed. Helicopter roller over. (.4)the pilot reported that he had just lifted off and attained an altitude of about 20 feet, when the engine experienced a loss of power. The pilot initiated an autorotation near the end of the airpark. The pilot stated that the helicopter was yawed slightly to the left on touchdown to a plowed muddy area. The right side landing skid collapsed and the helicopter rolled over. Inspection of the engine revealed that one of the two cylinders would not hold compression. Further inspection found evidence of overheating and damage to the rings on the piston. The pilot reported that the engine had been experiencing heating problems, and that he had also modified the carburetor. (-23)pilot departed Newberg, OR airport. At 20 ft. altitude the engine seized. Pilot autorotated into a plowed field, with approx. 20 crab angle. Rolled the helicopter to the right when the skid failed.
