From the FAA’s Accident/Incident Data System (AIDS), through its Aviation Safety Information Portal, as reported. The NTSB investigates accidents separately, and its record of an accident is the fuller one. The aircraft is matched to the registry by N-number and event date, checked against the reported make and model; the registry’s history has gaps, so older matches can be wrong. Compare the reported make and model.

What happened

The FAA’s narrative, in sentence case. The FAA writes it in capitals and removes private details.

(-5)Mr. Mayo was taking off from the Onawa (K36) airport in N427S on runway 15 when he lost directional control and the aircraft departed the runway to the left. The aircraft sustained minor damage to the nose gear, nose section, and the propeller. The pilot stated that he applied right rudder and right brake, with "no effect." There was a left crosswind of approximately 40 degrees at 13 knots. All of the witness reports verify the existance of a crosswind from the left at varing speeds. Maintenance reports no brake or steering problems. The pilot has been cooperative, but has limited experience in both total time and in this aircraft. This incident is considered closed with the report.
