From the FAA’s Accident/Incident Data System (AIDS), through its Aviation Safety Information Portal, as reported. The NTSB investigates accidents separately, and its record of an accident is the fuller one. The aircraft is matched to the registry by N-number and event date, checked against the reported make and model; the registry’s history has gaps, so older matches can be wrong. Compare the reported make and model.

What happened

The FAA’s narrative, in sentence case. The FAA writes it in capitals and removes private details.

Aircraft wheel caught in soft spot on runway on attempted take-off. Aircraft nosed over damaging propeller blades (3) and then fell back onto tail wheel. Damage to tail wheel reported. Engine and propeller removed and sent to repair station for repairs. Operator will report full extent of damage once assessment is done. I spoke with operator on 4/12/99. There was no other damage to aircraft. Engine and propeller were repaired, reinstalled and aircraft is now operational.
