From the FAA’s Accident/Incident Data System (AIDS), through its Aviation Safety Information Portal, as reported. The NTSB investigates accidents separately, and its record of an accident is the fuller one. The aircraft is matched to the registry by N-number and event date, checked against the reported make and model; the registry’s history has gaps, so older matches can be wrong. Compare the reported make and model.

What happened

The FAA’s narrative, in sentence case. The FAA writes it in capitals and removes private details.

Doing high speed taxi test. At approximately 30-35 KTS, 400 to 500 feet before the departure end of runway, became airborne and elected to try to fly around rather than immediately land. Nose wheel caught in 2 foot high alfalfa and aircraft nosed over, rotor blade struck ground 500 feet from the runway. Rotor separated from the aircraft and came to rest 150 feet from initial strike point. Aircraft continued to tumble and came to rest approximately 1000 feet from the departure end of the runway.
