From the FAA’s Accident/Incident Data System (AIDS), through its Aviation Safety Information Portal, as reported. The NTSB investigates accidents separately, and its record of an accident is the fuller one. The aircraft is matched to the registry by N-number and event date, checked against the reported make and model; the registry’s history has gaps, so older matches can be wrong. Compare the reported make and model.

What happened

The FAA’s narrative, in sentence case. The FAA writes it in capitals and removes private details.

(-23) LH outboard main tire separated from aircraft after take-off. Inspection of tire indicated axle bolt had disengaged from axle threads. The landing gear was found to have the anti skid adapter assembly installed at 13/16" instead of the required dept of 1 and 3/32" or greater. This position allowed the axle bolt to be installed with clearance between the spacer and axle end. This clearance prevented the proper engagement of the anti-rotation device in the axle groove and resulted in the loss of the tire assembly.

As the FAA filed it
