From the FAA’s Accident/Incident Data System (AIDS), through its Aviation Safety Information Portal, as reported. The NTSB investigates accidents separately, and its record of an accident is the fuller one. The aircraft is matched to the registry by N-number and event date, checked against the reported make and model; the registry’s history has gaps, so older matches can be wrong. Compare the reported make and model.

What happened

The FAA’s narrative, in sentence case. The FAA writes it in capitals and removes private details.

(-23) Departed runway 31 at dsm with thunderstorms in the vicinity. After take off (flaps 8), we commenced a left turn to avoid the storms. At acceleration altitude, the flaps were selected to (0) deg. And we immediately received a flaps fail and the associated cautions. The flaps were stuck @ 8 deg. We followed the cfm procedures, called dispatch and ops, leveled at 10,000 ft. we told dsm app that we would be returning to dsm. There was a break in the thunderstorms and even with an over weight landing it was decided to return. Landing was smooth and uneventful.
