From the FAA’s Accident/Incident Data System (AIDS), through its Aviation Safety Information Portal, as reported. The NTSB investigates accidents separately, and its record of an accident is the fuller one. The aircraft is matched to the registry by N-number and event date, checked against the reported make and model; the registry’s history has gaps, so older matches can be wrong. Compare the reported make and model.

What happened

(-23) On 09/23/01, at 1414E, at Syracuse Hancock International Airport (syr), in Syracuse, NY, a jetstream 4101, N55HK, serial no. 41075, operated by trans states airlines, inc. (raia), flight number 5409. While taxiing for departure, the passengers and crew did an emergency evacuation via the right rear emergency exit due to a fire warning indication on the number one engine. They stopped on taxiway "a", at the intersection of taxiway "b". The crew used both fire bottles. The ARFF responded but did not have to use their equipment. The passengers were bussed back to the gate and there were no injuries reported. The aircraft was towed back to the ramp at piedmont hawthorne. The rochester FSDO and NTSB were notified by the com center. The flight was to depart to ewr. The weather was VFR, visibility 10 miles, few at 45, sct at 250 and temp 23 dew point 12. Later that same day, 2 mechanics from trans stated airlines, inc. were flown in to look at the aircraft. They found the insulation on the exhaust for the left engine was peeled back, which exposed the metal exhaust pipe. The fire detection loop wires are located close to the exhaust system. They believed this was the problem. A new exhaust pipe and insulation were installed and a run up was performed with no problems noted.
