From the FAA’s Accident/Incident Data System (AIDS), through its Aviation Safety Information Portal, as reported. The NTSB investigates accidents separately, and its record of an accident is the fuller one. The aircraft is matched to the registry by N-number and event date, checked against the reported make and model; the registry’s history has gaps, so older matches can be wrong. Compare the reported make and model.

What happened

The FAA’s narrative, in sentence case. The FAA writes it in capitals and removes private details.

(-23) On June 19, 2002, at 1744 hours EDT, a Beech starship 2000, N786BP, registered to 86 bp, inc, experienced the loss of a main landing gear wheel while on a part 91 flight. Visual meteorological conditions prevailed at the time. The aircraft sustained no damage and the airline transport-rated pilot and his instructor were not injured. The flight originated at fort Myers, Florida at approximately 1740 hours. The aircraft had made two full stop landings at fort Myers, Florida (fmy) and on the third takeoff the tower advised the crew that they had lost a wheel/tire assembly. The aircraft landed without incident. The wheel bearings had disintegrated in the right inboard wheel assembly (#3), allowing the wheel to move inboard over the retaining nut and depart the aircraft. Inspection of the other three main wheels revealed that the bearing races in #1 wheel had been overheated and were turning blue. The races in #4 wheel showed signs of spalling. The races in the #2 wheel appeared normal. The Beech service information requires a bearing lubrication interval of 100 hours. The subject bearings were inspected and lubricated 71 hours prior to failure. #3 wheel bearing total time in service was 346 hours and 25 months.
