From the FAA’s Accident/Incident Data System (AIDS), through its Aviation Safety Information Portal, as reported. The NTSB investigates accidents separately, and its record of an accident is the fuller one. The aircraft is matched to the registry by N-number and event date, checked against the reported make and model; the registry’s history has gaps, so older matches can be wrong. Compare the reported make and model.

What happened

The FAA’s narrative, in sentence case. The FAA writes it in capitals and removes private details.

(-23) Upon approaching the landing site pilot requested the landing checklist. The co-pilot notified the pilot that there was no indication of gear "in-transit" or gear down lights. The gear level was recycled but still no indication. Pilot requested the "emergency landing gear extension" and after several attempts they still do not have any gear down or transit lights. Upon arriving at Dulles airport maintenance technician attempted to manually release the gear without any success. Pilot proceeded to land the helicopter with the gear up. Helicopter was secured on the ground.
