From the FAA’s Accident/Incident Data System (AIDS), through its Aviation Safety Information Portal, as reported. The NTSB investigates accidents separately, and its record of an accident is the fuller one. The aircraft is matched to the registry by N-number and event date, checked against the reported make and model; the registry’s history has gaps, so older matches can be wrong. Compare the reported make and model.

What happened

The FAA’s narrative, in sentence case. The FAA writes it in capitals and removes private details.

(-23) Flying boat inc. flight 501 was on a flight from pid (paradise island-bahamas) to fll (ft. Lauderdale, Florida) when the crew noticed hydraulic quantity decreasing. On approach to fll crew followed emergency gear extension procedures, verified gear down and locked. Also requested and received gear down verification from fll control tower. Upon landing, main gear contacted runway with no problem. Nose gear collapsed, aircraft skidded down runway, then left main gear collapsed. Aircraft continuied skidding down runway veering off to left, and coming to rest off left side of runway 07R. Passengers and crew evacuated aircraft with no injuries. Investigation revealed the following: "o" ring seal on right main landing gear down lock actuator popped out, which caused loss of hydraulic fluid. Reason "o" ring popped out was worn actuator piston, allowing for excessive clearance between pistgon, and cylinder. Down lock actuator overhual manual was checked for tolerance limits. Piston met limits outlined in overhaul manual. To prevent this from happening again, flying boat has initiated two steps. Step one is to take all available blue prints for actuator, and have their FAA designated engineer representative initiate steps for flying boat to manufacture new actuators. Step two is examine all spare actuators for tolerance limits, and reject units that area close to existing limits. As new actuators become available, they will replace existing units installed on aircraft.

As the FAA filed it
