From the FAA’s Accident/Incident Data System (AIDS), through its Aviation Safety Information Portal, as reported. The NTSB investigates accidents separately, and its record of an accident is the fuller one. The aircraft is matched to the registry by N-number and event date, checked against the reported make and model; the registry’s history has gaps, so older matches can be wrong. Compare the reported make and model.

What happened

The FAA’s narrative, in sentence case. The FAA writes it in capitals and removes private details.

(-23) The CFI was giving the first flight lesson to a student. The CFI took off, flew a traffic pattern and landed to the middle of the sod runway. The CFI turned the helicopter into the wind (200 at 9 knots), and proceeded to instruct the student. The CFI told the student to take the collective and hold the same altitude while the CFI held the aircraft in the same position. The CFI had the student remain lightly on the controls but told him to concentrate on the collective. The student made an unexpected rapid downward movement with the collective and simultaneously introduced a substantial left cyclic input. The CFI attempted to correct, but the helicopter began a left dynamic rollover before the CFI could gain control. In an interview with the student, he stated he sensed the helicopter made a bank to the left and he bottomed the collective at that time. He also stated he yanked the cyclic to the right and felt he had jerked it from the cfi's hand.
