From the FAA’s Accident/Incident Data System (AIDS), through its Aviation Safety Information Portal, as reported. The NTSB investigates accidents separately, and its record of an accident is the fuller one. The aircraft is matched to the registry by N-number and event date, checked against the reported make and model; the registry’s history has gaps, so older matches can be wrong. Compare the reported make and model.

What happened

The FAA’s narrative, in sentence case. The FAA writes it in capitals and removes private details.

(-23) While on a flight originating from Venice Municipal Airport the pilot heard a loud grinding noise in flight. A precautionary landing was made at sorento shores and while operating the helicopter at 100% on the ground, the cooling fan assembly shaft broke. This allowed the rotating fan assy to impact the shroud assembly causing damage. Investigation did not indicate any improper maintenance or operation. A malfunction defect report (FAA form 8010-4) was filed on the cooling fan shaft p/n A642-2.
