From the FAA’s Accident/Incident Data System (AIDS), through its Aviation Safety Information Portal, as reported. The NTSB investigates accidents separately, and its record of an accident is the fuller one. The aircraft is matched to the registry by N-number and event date, checked against the reported make and model; the registry’s history has gaps, so older matches can be wrong. Compare the reported make and model.

What happened

The FAA’s narrative, in sentence case. The FAA writes it in capitals and removes private details.

(-23) On September 7, 2003, at approximately 1028 LCL, a Cessna 180D on floats, N6445X, piloted by michael j. conaboy, certificate number 2089588, com sel/ses & ATP MEL, at an altitude of approximately 100 feet AGL during climb after takeoff from moosehead lake, experienced loss of aileron control. Attempts to control aircraft using power, rudder and elevator were not effective and aircraft struck water in a right wing low and nosedown attitude causing it to cartwheel. Aircraft subsequently came to rest upside down with cabin submerged. CFI conaboy, acting as PIC occupied the right seat and was giving Mr. Frederick schick, certificate number 35308699, private pilot with sel/ses/inst ratings, a currency checkout. Both occupants exited the aircraft with only minor injuries and were rescued by a nearby boater. Aircraft received substantial damage.
