From the FAA’s Accident/Incident Data System (AIDS), through its Aviation Safety Information Portal, as reported. The NTSB investigates accidents separately, and its record of an accident is the fuller one. The aircraft is matched to the registry by N-number and event date, checked against the reported make and model; the registry’s history has gaps, so older matches can be wrong. Compare the reported make and model.

What happened

The FAA’s narrative, in sentence case. The FAA writes it in capitals and removes private details.

(-23) At 1528 PST, 11/06/03, at toa airport, there was a fatal mid air collision between a R-44 (N442RH) and R-22 (N206TV) helicopter. The collision occurred above runway 29L. The R-44 was on an instructional flight and had been given clearance for the option on 29L. Reports indicated it was traveling west and climbing. N206TV was operated by a student pilot with 1.5 previous solo hours. On this third solo flight, he had been practicing at the north pad (toa). After completing right traffic pattern work at the north pad, he requested to and landing clearance for pch parking from the north pad. This was requested twice due to heavy traffic. He was then instructed by the tower to fly westbound. The controller then instructed N206TV to make a right turn to the downwind. The controller later informed him that he would cross him at midfield as soon as he had a chance. After he was downwind the tower controller instructed him to turn right. A short time later the tower controller informed him "runway 29R cleared to land". Shortly after this the tower controller again stated,"turn right helicopter 6TV, runway 29R cleared to land". The collision occurred moments later.

As the FAA filed it
