From the FAA’s Accident/Incident Data System (AIDS), through its Aviation Safety Information Portal, as reported. The NTSB investigates accidents separately, and its record of an accident is the fuller one. The aircraft is matched to the registry by N-number and event date, checked against the reported make and model; the registry’s history has gaps, so older matches can be wrong. What happened

The FAA’s narrative, in sentence case. The FAA writes it in capitals and removes private details.

(-23) Ata airlines was operating "reach" flt. 4142, from north Island naval air station, coronado, CA. During the starting sequence the aircraft rolled forward causing the right main landing gear assembly to push a ground air cart with a tow tractor attached to it into the number three engine cowling. Damage consisted of crushing of the lower cowling inlet leading edge, and minor sheel metal scrapping on the lower left fan cowling asssembly. There were no injuries to any of the ground crew, flight crew and/or passengers. This incident was caused in part by a miscommunication between the ground crew and the mechanic who was signalling the flight crew. The ground crew and mechanic were not wearing headsets, and relied solely on hand signal that were being given by the flight crew. The flight crew indicated that the brakes were set, but for unknown reasons, were not engaged. A functional test was made on the brake system by ata maintenance officials. The brakes test okay, with MO mechanical abnormalities being noted.
