From the FAA’s Accident/Incident Data System (AIDS), through its Aviation Safety Information Portal, as reported. The NTSB investigates accidents separately, and its record of an accident is the fuller one. The aircraft is matched to the registry by N-number and event date, checked against the reported make and model; the registry’s history has gaps, so older matches can be wrong. Compare the reported make and model.

What happened

The FAA’s narrative, in sentence case. The FAA writes it in capitals and removes private details.

(-23) On may 12, 2004 gary matthys was PIC of N2301F (Robinson R-22 beta ii helicopter) at approximately 700 feet AGL near Winsted, MN when he heard a loud snap, then smelled burning rubber and the clutch light illuminated. Mr. Mathys assumed he was losing and/or had lost the drive belts. He immediately initiated an autorotation and impacted the ground destroying the aircraft. Mr. Mathys sustained no serious injuries. Upon inspection of the aircraft it is suspected that one of the v-belts failed causing both v-belts to come off the sheaves forcing Mr. Mathys to make an emergency landing.
