From the FAA’s Accident/Incident Data System (AIDS), through its Aviation Safety Information Portal, as reported. The NTSB investigates accidents separately, and its record of an accident is the fuller one. The aircraft is matched to the registry by N-number and event date, checked against the reported make and model; the registry’s history has gaps, so older matches can be wrong. Compare the reported make and model.

What happened

The FAA’s narrative, in sentence case. The FAA writes it in capitals and removes private details.

(-23) Aircraft exited the runway on the left side proceeded at that time to collapse the nose gear, aircraft then crossed the runway and came to rest on the right side of the runway. Investigation to continue as to why the aircraft exited the runway. (-23 amendment) on 06/30/2004 an inspection to the aircraft was performed with the following results: 1) no malfunctions or defects were noted on either the left or right main landing gear brake assemblies. 2) the nose landing gear strut assembly has evidence that the nose tire was at a 10 to 15 degree left turn as damage to the strut assembly occurred. No specific part or component could be identified that may have caused or contributed to that left turn. The pilot report of an uncommented left turn could not be verified through the inspection of the aircraft or related landing gear components.
