From the FAA’s Accident/Incident Data System (AIDS), through its Aviation Safety Information Portal, as reported. The NTSB investigates accidents separately, and its record of an accident is the fuller one. The aircraft is matched to the registry by N-number and event date, checked against the reported make and model; the registry’s history has gaps, so older matches can be wrong. Compare the reported make and model.

What happened

The FAA’s narrative, in sentence case. The FAA writes it in capitals and removes private details.

(-23) Catastrophic failure of the throttle bellcrank/throttle cable bearing, throttle control became inoperative upon bearing failure. Pilot performed a successful running landing. Inspection of the throttle bellcrank revealed that the bearing linking the throttle control cable to the throttle bellcrank had disintegrated resulting in a total loss of rotorcraft throttle control. A mechanic commented that the throttle cable was mounted on the wrong side of the bellcrank assembly. There was also evidence of wear on the bellcrank adjacent to where the throttle cable was installed. Bearing may have failed due to improper routing of the throttle cable linkage.
