From the FAA’s Accident/Incident Data System (AIDS), through its Aviation Safety Information Portal, as reported. The NTSB investigates accidents separately, and its record of an accident is the fuller one. The aircraft is matched to the registry by N-number and event date, checked against the reported make and model; the registry’s history has gaps, so older matches can be wrong. Compare the reported make and model.

What happened

The FAA’s narrative, in sentence case. The FAA writes it in capitals and removes private details.

(-23) Aircraft departed Jackson county airport on runway 01, during climb out, engine began to sputter and was losing power. While turning back to airport, engine quit and pilot landed short of airport in a pasture field. After landing, aircraft slid into trees, tore its right wing off and came to rest in a creek bed on its right side. Aircraft has substantial damage and pilot has minor injuries. Pilot stated that he had right fuel tank selected and it was full before flight. Left tank was 1/4 full. The Ohio state highway patrol reported no odor of fuel at accident scene. Unable to determine the amount of fuel in aircraft after the accident, because damage to right wing and position of aircraft in creek bed. Pilot stated previous to accident, engine had lost power on take off, but thought problem was now fixed.
