From the FAA’s Accident/Incident Data System (AIDS), through its Aviation Safety Information Portal, as reported. The NTSB investigates accidents separately, and its record of an accident is the fuller one. The aircraft is matched to the registry by N-number and event date, checked against the reported make and model; the registry’s history has gaps, so older matches can be wrong. Compare the reported make and model.

What happened

The FAA’s narrative, in sentence case. The FAA writes it in capitals and removes private details.

(-23) Pilot lifted off from pad, climbed to approximately 20 feet and aircraft started to lose power. Pilot performed a right hand descending turn into an open field adjacent and 1000 feet west of departure point. Pilot attempted run-on landing and touched down with full left pedal and nose right position. Left skid struck small berm and aircraft proceeded into dynamic roll-over vent. Minor airframe damage and substantial rotor system damage occurred as a result. Pilot encountered 4 fractured ribs and was initially pinned under the collective control when aircraft rolled onto the left side and had to be extracted by emergency services personnel. Right seat passenger was uninjured during the event.
