From the FAA’s Accident/Incident Data System (AIDS), through its Aviation Safety Information Portal, as reported. The NTSB investigates accidents separately, and its record of an accident is the fuller one. The aircraft is matched to the registry by N-number and event date, checked against the reported make and model; the registry’s history has gaps, so older matches can be wrong. Compare the reported make and model.

What happened

The FAA’s narrative, in sentence case. The FAA writes it in capitals and removes private details.

(-23) On October 08, 2004 world airways attempted to perform a 2 engine ferry flight with the center gear retracted. When the captain released the brakes at the start of the takeoff run, the aircraft went on its tail and sustained major damage to the belly skin and internal structure. The NTSB has declared this as an accident and the preliminary finding is that with the center gear retracted there is a significant CG change and the crew did not have the appropriate information in their manuals and dispatch paperwork regarding takeoff with the center gear retracted. Reference the NTSB preliminary report ANC05LA002 which will be available the week of October 18-22, 2004. (-23 amendment) amended 10-21-2004---THE incorrect pilot names have been removed from this form and the correct pilot names will be added on the next amendment. Amended 11-04-2004---THE correct pilot names have been added to this form.
