From the FAA’s Accident/Incident Data System (AIDS), through its Aviation Safety Information Portal, as reported. The NTSB investigates accidents separately, and its record of an accident is the fuller one. The aircraft is matched to the registry by N-number and event date, checked against the reported make and model; the registry’s history has gaps, so older matches can be wrong. Compare the reported make and model.

What happened

The FAA’s narrative, in sentence case. The FAA writes it in capitals and removes private details.

(-23) Instructor and student were climbing out in an R-44 helicopter to practice right traffic maneuvers at Linden Airport. Passing 200' the student said he felt a vibration, then the "low rotor/rpm horn and light" came on. The instructor took control, sensed the loss of power, and auto rotated into a vacant construction site. The site was muddy and the leading edges of the skids dug into the turf, rocking the craft fwd and back. The turning/trailing rotor blades contacted the rear boom and virtually severed it (mast dampening). No damage to persons or other property. It is viewed the instructor acted prudently. No enforcement action planned.
