From the FAA’s Accident/Incident Data System (AIDS), through its Aviation Safety Information Portal, as reported. The NTSB investigates accidents separately, and its record of an accident is the fuller one. The aircraft is matched to the registry by N-number and event date, checked against the reported make and model; the registry’s history has gaps, so older matches can be wrong. Compare the reported make and model.

What happened

The FAA’s narrative, in sentence case. The FAA writes it in capitals and removes private details.

(-23) Aircraft departed ADS, encountered engine problems. Requested return to ADS. The number 2 engine stopped running. Shut down and secured engine. On landing had three green. Touched down and on roll out nose gear collapsed. Left engine prop struck the runway. Damage to fuselage skin adjacent to nose gear area, and left engine. Found that a fire had started in the area of the right engine fuel pump, burning lines and wires. The fire probably extinguished when the engine shutdown. Found the upper fitting on the fuel hose from fuel pump to injector servo loose about 1/2 turn. This line is directly above the exhaust, and possible cause of fire/ignition. No maintenance performed on this aircraft in the last week. Aircraft hobbs: 0890.3, aircraft hobbs at takeoff: 0890.1. Aircraft total time: 8075.5. Ran an SDR report. No findings for engine problem or landing gear for a smith aircraft. Only 1 SDR reported for this mfg, none for this model.
