From the FAA’s Accident/Incident Data System (AIDS), through its Aviation Safety Information Portal, as reported. The NTSB investigates accidents separately, and its record of an accident is the fuller one. The aircraft is matched to the registry by N-number and event date, checked against the reported make and model; the registry’s history has gaps, so older matches can be wrong. Compare the reported make and model.

What happened

The FAA’s narrative, in sentence case. The FAA writes it in capitals and removes private details.

(-23) C-fqpk, a PA-30 enroute from waterloo regional airport (ykf) to teterboro (teb) reported icing at 6000 feet over huegonot VOR, and was cleared to descend. During the descent, the left engine began surging, and eventually failed. The engine was restarted and the aircraft continued to teb. The PA-30 airplane flight manual prohibits the use of the tip tanks during descent, however, the pilot/owner of the aircraft stated he had descended while using tip tanks on prior flights with no problems.
