From the FAA’s Accident/Incident Data System (AIDS), through its Aviation Safety Information Portal, as reported. The NTSB investigates accidents separately, and its record of an accident is the fuller one. The aircraft is matched to the registry by N-number and event date, checked against the reported make and model; the registry’s history has gaps, so older matches can be wrong. Compare the reported make and model.

What happened

The FAA’s narrative, in sentence case. The FAA writes it in capitals and removes private details.

(-23) Helicopter departed dekalb airport (dkb) for a local flight. At approximately 7 miles west of dkb a yellow engine warning light came on the fadec display indicating a code #2 warning and the pilot decided to make a precautionary landing in a corn field 1 1/2 miles west of Malta, IL. It was determined that the fadec code was triggered by a momentary overspeed of engine RPM and after letting water and oil temperatures stabilize, the pilot attempted to takeoff. As pilot raised the collective, the right skid rose off the ground but the left skid was stuck in the mud and the helicopter abruptly rolled over onto the left skid and continued over until the main rotor blades contacted the ground. The helicopter received substantial damage and the pilot and passenger egressed through the broken windshield. The female passenger was taken to a local hospital as a precaution and later released. The pilot received only a minor scratch on his chin.

As the FAA filed it
