From the FAA’s Accident/Incident Data System (AIDS), through its Aviation Safety Information Portal, as reported. The NTSB investigates accidents separately, and its record of an accident is the fuller one. The aircraft is matched to the registry by N-number and event date, checked against the reported make and model; the registry’s history has gaps, so older matches can be wrong. Compare the reported make and model.

What happened

The FAA’s narrative, in sentence case. The FAA writes it in capitals and removes private details.

(-23) Shortly after take off from rwy 264, and gaining an altitude of @ 50 feet, the elevator controls jammed. The aircraft descended back on the runway. The hard landing caused damage to the main landing gear, and the prop striking the runway. After a 400 feet skid the aircraft came to a stop. Aircraft was towed to the pilot's hangar. My inspection in the aircraft cockpit revealed a handheld radio jammed in the flight control bellcrank. The control stick and bellcrank is located on the right side (@ 5 o'clock) of the cockpit. The pilot stated that the radio slipped from his lap during take off roll.
