From the FAA’s Accident/Incident Data System (AIDS), through its Aviation Safety Information Portal, as reported. The NTSB investigates accidents separately, and its record of an accident is the fuller one. The aircraft is matched to the registry by N-number and event date, checked against the reported make and model; the registry’s history has gaps, so older matches can be wrong. Compare the reported make and model.

What happened

The FAA’s narrative, in sentence case. The FAA writes it in capitals and removes private details.

(-23) Aircraft just came out of 100 hr annual inspection. Preinspection ck list shows l/h tank empty r/h tank 1/2. Fuel selector valve handle and position indication cover was removed to facilitate maintenance. At time of removal handle was located so that the l/h tank was open. When it was reinstalled the handle was put on 180 degree off and pointing to r/h tank open. When pilot prepared to fly he turned selector to the both position, but because selector handle was 180 degrees off, it actually turned fuel off.
