From the FAA’s Accident/Incident Data System (AIDS), through its Aviation Safety Information Portal, as reported. The NTSB investigates accidents separately, and its record of an accident is the fuller one. The aircraft is matched to the registry by N-number and event date, checked against the reported make and model; the registry’s history has gaps, so older matches can be wrong. Compare the reported make and model.

What happened

The FAA’s narrative, in sentence case. The FAA writes it in capitals and removes private details.

(-23) The aircraft crashed on take-off from montague island about 5 miles north east of beach river. According to the pilot, he lost control of the aircraft and ran off the end of the runway striking trees. The beach he was departing from used to be part of the main coastline of the island before the 1964 earthquake. It is now overgrown and about 600 feet long. The pilot said he goes out each year and clears some of the brush from the landing area. The pilot admitted that he had not flown much this year, and that is the reason for the accident.
