From the FAA’s Accident/Incident Data System (AIDS), through its Aviation Safety Information Portal, as reported. The NTSB investigates accidents separately, and its record of an accident is the fuller one. The aircraft is matched to the registry by N-number and event date, checked against the reported make and model; the registry’s history has gaps, so older matches can be wrong. Compare the reported make and model.

What happened

The FAA’s narrative, in sentence case. The FAA writes it in capitals and removes private details.

(-23) During take off from ont (repositioning flight to san), the engine indication and crew warning system "smoke in the lavatory" message illuminated. The crew declared an emergency and diverted back to ont. Upon landing, the fire department discovered no evidence of fire or smoke. The air carrier's time control index (inspection task cards) do not have an inspection task for the lavatory smoke sensor. Corrective action: I contacted the primary maintenance inspector to coordinate with the air carrier, a change to the inspection task cards to include: checking the lavatory sensor for dirt/dust buildup and removal of the buildup if required.
