From the FAA’s Accident/Incident Data System (AIDS), through its Aviation Safety Information Portal, as reported. The NTSB investigates accidents separately, and its record of an accident is the fuller one. The aircraft is matched to the registry by N-number and event date, checked against the reported make and model; the registry’s history has gaps, so older matches can be wrong. Compare the reported make and model.

What happened

The FAA’s narrative, in sentence case. The FAA writes it in capitals and removes private details.

(-23) On 5-26-08 the aircraft departed lyons rice county airport (lyo) with approximately 170 gallons of chemical onboard to perform an aerial application. Shortly after departure the aircraft experienced loss of power and made an off airport landing without incident. The aircraft was towed back to lyo where maintnenance replaced the magnetos, spark plugs and checked the fuel system. The aircraft was ground run and flight tested with no discrepanies. On 6-12-08 the aircraft departed lyo loaded with 150 gallons of chemical onboard to perform an aerial application. After takeoff the pilot circled the airport for several passes with no discrepancies noted. The pilot departed the airport to the east and after approximately 1 1/2 miles the engine again lost power. The pilot stated that he thought that the engine was going to seize completely. An off airport landing was executed and during the landing roll out the aircraft struck a waterway ditch embankment removing the left landing gear leg from the airframe in the center section of the fuselage. The aircraft was removed from the accident site to lyo.
