From the FAA’s Accident/Incident Data System (AIDS), through its Aviation Safety Information Portal, as reported. The NTSB investigates accidents separately, and its record of an accident is the fuller one. The aircraft is matched to the registry by N-number and event date, checked against the reported make and model; the registry’s history has gaps, so older matches can be wrong. Compare the reported make and model.

What happened

The FAA’s narrative, in sentence case. The FAA writes it in capitals and removes private details.

(-23) Shortly after takeoff, the nr 4 engine fire warning light illuminated, and the nr 4 engine nacelle temp gauge indicated into the red zone. The crew discharged the nr 4 fire bottle, dumped fuel, and returned safely to kvcv. There was no emergency evacuation of the crew and military personnel (contract flight for the us army). Maintenance discovered a ruptured 13th stage cooling air duct and high pressure hot air (no actual fire) damage to the nr 4 engine right hand core cowl. After replacing the duct and cowl, the engine was operationally checked ok. The aircraft was then placed back into service.

AFTER REPLACING THE DUCT AND COWL, THE ENGINE WAS OPERATIONALLY CHECKED OK. THE AIRCRAFT WAS THEN PLACED BACK INTO SERVICE.
