From the FAA’s Accident/Incident Data System (AIDS), through its Aviation Safety Information Portal, as reported. The NTSB investigates accidents separately, and its record of an accident is the fuller one. The aircraft is matched to the registry by N-number and event date, checked against the reported make and model; the registry’s history has gaps, so older matches can be wrong. Compare the reported make and model.

What happened

The FAA’s narrative, in sentence case. The FAA writes it in capitals and removes private details.

(-23) During routine instructional flight with a CFI and student, the nose gear collapsed upon landing while practicing touch and goes at gyr. Post incident investigation found that the gear showed signs of damage and deterioration prior to the incident. A review of the aircraft records showed that the requirements of AD 2005-13-16 and Piper service bulletin (sb) 1123A, have not been accomplished. The AD requires repetitive inspections that have not been accomplished at the intervals stated in the sb. The lack of inspections may have been a causal factor in the incident.
