From the FAA’s Accident/Incident Data System (AIDS), through its Aviation Safety Information Portal, as reported. The NTSB investigates accidents separately, and its record of an accident is the fuller one. The aircraft is matched to the registry by N-number and event date, checked against the reported make and model; the registry’s history has gaps, so older matches can be wrong. Compare the reported make and model.

What happened

The FAA’s narrative, in sentence case. The FAA writes it in capitals and removes private details.

(-23) The helicopter was being flown for the purpose of conducting an instructional flight. The owner/student had flown for approximately 15 minutes and made two landings. Following a third liftoff, a hover was established, and the helicopter began to accelerate westbound. A climb was initiated after achieving 45 mph, and at approximately 40 feet AGL, the engine lost power. The instructor took the controls, attempted an autorotation, but was unable to arrest the rate of descent completely. As the helicopter impacted the ground, it sustained substantial damage, but the occupants sustained only minor injuries.
