From the FAA’s Accident/Incident Data System (AIDS), through its Aviation Safety Information Portal, as reported. The NTSB investigates accidents separately, and its record of an accident is the fuller one. The aircraft is matched to the registry by N-number and event date, checked against the reported make and model; the registry’s history has gaps, so older matches can be wrong. Compare the reported make and model.

What happened

The FAA’s narrative, in sentence case. The FAA writes it in capitals and removes private details.

The anchorage FSDO incident number is:IAL0320090049. Nose gear failed to extend prior to landing. At tpo pilot attempted to extend gear normally and later by emergency means, but nose gear would not extend. Pilot conferred with maintenance staff and tried various means to extend nose gear. All attempts were unsucessful. Pilot finally determined to make partial gear-up landing. He instructed passengers to move aft in cabin, lined up aircraft with runway. Then on short final, shut down both engines and feathered both propellers. Touched down smoothly and held nose off of ground for as long as possible. After coming to a complete stop, all occupants evacuated with no injuries. Initial evaluation of damage to aircraft appears to be mostly to nose landing gear doors, which took majority of contact with runway. There appears to be no evidence of propeller strike on either left or right side. Spoke with d.o.m. privacy data omitted On sept 3, 2009 by phone. Status of aircraft was still a.o.g. for parts. Landing gear electrical contactor box also out for evaluation and repair. Spoke with d.o.m. privacy data omitted Again on sept 4, 2009 by phone. Requested copies of documentation for past work on aircraft landing gear, and for certs and traceability. Emphasized traceability of NLG actuator that was installed 11/1/08 serviceable with 396 cycles since overhaul. Post-incident photos show NLG actuator lower end and clevis sheared off. 09-15-2009: spoke with the d.o.m. privacy data omitted. Additional information regarding the nose gear actuator (nga) s/n 658 was requested. The nga is a "on condition" part. The nga was installed on 11-01-2008 as a serviceable used part with 396 cycles since overhaul on it. This nga was overhauled on 05-18-2001 and a authorized release certificate number WO-W7292 under work order number W5299 from transport Canada was provided. This nga requires a external inspection and end play check at 1000 cycles or 30 months. This inspection was performed by the operator at the time of installation on 11-01-2008. Since installation of this nga the aircraft has accumulated an additional 301 cycles for a total of 697 cycles on the nga since overhaul. The d.o.m. theorizes the motor controller allowed the nose landing gear to retract to FAR causing damage to the nga and the subsequent failure of the nga to extend for landing. The motor controller has been sent to a facility in Kansas for inspection and overhaul. This aircraft is still in a non-flying status awaiting parts. This entry dated 09-15-2009 made by inspector klm. Previous entries and the investigation has been accomplished by inspector rmg.

As the FAA filed it

^PRIVACY DATA OMITTED^ ON SEPT 3, 2009 BY PHONE. STATUS OF AIRCRAFT WAS STILL A.O.G. FOR PARTS. LANDING GEAR ELECTRICAL CONTACTOR BOX ALSO OUT FOR EVALUATION AND REPAIR. SPOKE WITH D.O.M. ^PRIVACY DATA OMITTED^ AGAIN ON SEPT 4, 2009 BY PHONE. ^PRIVACY DATA OMITTED^. THE MOTOR CONTROLLER HAS BEEN SENT TO A FACILITY IN KANSAS FOR INSPECTION AND OVERHAUL.THIS AIRCRAFT IS STILL IN A NON-FLYING STATUS AWAITING PARTS.THIS ENTRY DATED 09-15-2009 MADE BY INSPECTOR KLM. PREVIOUS ENTRIES AND THE INVESTIGATION HAS BEEN ACCOMPLISHED BY INSPECTOR RMG.
