From the FAA’s Accident/Incident Data System (AIDS), through its Aviation Safety Information Portal, as reported. The NTSB investigates accidents separately, and its record of an accident is the fuller one. The aircraft is matched to the registry by N-number and event date, checked against the reported make and model; the registry’s history has gaps, so older matches can be wrong. Compare the reported make and model.

What happened

The FAA’s narrative, in sentence case. The FAA writes it in capitals and removes private details.

(-23) On August 9, 2009, the pilot privacy data omitted, a PA-28R-200, attempted a takeoff from soldier meadows (NV06). The aircraft was approximately 175 lbs under the gross weight of 2650 lbs. During the takeoff roll, the aircraft drifted to the right side of the runway and continued down the right side in approximately 6 inches of weeds. After 2790 feet of takeoff roll, the aircraft ran over a series of white painted truck tires on the right side of the runway and then swerved left, crossing the runway, and departing the left side causing the nose and left main landing gear to collapse. Substantial damage was incurred to both wings, landing gear, empennage, and horizontal stabilizer. Aeronautical charts show the strip to be 4000 feet in length. Actural measurement of the strip revealed that 3090 feet were usable to the white tire markers, with another 1041 feet available to the fence resulting in a total distance of 4131 feet. Takeoff was initiated with 300 feet of runway behind him and only 2790 feet left to the white tire markers used to presumably mark the end of the runway.
