From the FAA’s Accident/Incident Data System (AIDS), through its Aviation Safety Information Portal, as reported. The NTSB investigates accidents separately, and its record of an accident is the fuller one. The aircraft is matched to the registry by N-number and event date, checked against the reported make and model; the registry’s history has gaps, so older matches can be wrong. Compare the reported make and model.

What happened

The FAA’s narrative, in sentence case. The FAA writes it in capitals and removes private details.

A video of the event taken from a ual camera in the F2 gate area and a statement from the truck driver revealed the following:the driver was called to the F2D gate area to open a personnel access door. He drove the truck up to the building with the front of the truck facing the building, put the truck in park and got out leaving the engine running. He opened the door, retrieved a telephone number for an individual and went back to the truck only to find it was no longer there. In the video, the truck goes out of view for approximately 20 seconds as it approached the f concourse. When it appears again, it is moving in reverse. At the same time the swia aircraft is sitting, pushed back from gate F2A as the crew completes the pre-taxi checklist. As the crew begins to taxi the aircraft they notice the ual truck moving in reverse toward the nose of the aircraft on the pilot's side. The pilot stopped the aircraft and the truck struck the aircraft on the pilot side just forward of the nose landing gear.

HE OPENED THE DOOR, RETRIEVED A TELEPHONE NUMBER FOR AN INDIVIDUAL AND WENT BACK TO THE TRUCK ONLY TO FIND IT WAS NO LONGER THERE.IN THE VIDEO, THE TRUCK GOES OUT OF VIEW FOR APPROXIMATELY 20 SECONDS AS IT APPROACHED THE F CONCOURSE.
