From the FAA’s Accident/Incident Data System (AIDS), through its Aviation Safety Information Portal, as reported. The NTSB investigates accidents separately, and its record of an accident is the fuller one. The aircraft is matched to the registry by N-number and event date, checked against the reported make and model; the registry’s history has gaps, so older matches can be wrong. Compare the reported make and model.

What happened

The FAA’s narrative, in sentence case. The FAA writes it in capitals and removes private details.

12/2011 At 08:40 a landing aircraft reported a green metal piece of an aircraft on rwy 18L. This piece of metal was identified as a part of the thrust reverser system. This part is a cascade panel. The part was whole and had separated at the attachment points. In order to locate the affected aircraft, inspections were performed on aircraft that landed between when the last runway inspection performed at 07:30, and when the report came in at 08:40. Aircraft that landed during this time frame were inspected. An inspection on aircraft N406US which landed at 08:36 had a missing cascade panel. A new cascade panel was installed and a minor sheet metal repair on the cowling was performed.
