From the FAA’s Accident/Incident Data System (AIDS), through its Aviation Safety Information Portal, as reported. The NTSB investigates accidents separately, and its record of an accident is the fuller one. The aircraft is matched to the registry by N-number and event date, checked against the reported make and model; the registry’s history has gaps, so older matches can be wrong. Compare the reported make and model.

What happened

The FAA’s narrative, in sentence case. The FAA writes it in capitals and removes private details.

A witness reported a loss of control just after rotation. The aircraft yawed to the left and departed the paved surface of runway 10, at kgvq (loss of control occurred approximately 1000 feet from the threshold of runway 10) the aircraft continued in a north easterly direction, until contacting a grass berm approximately six feet in height. The aircraft came to rest in a inverted postion on top of the berm. The impact forces imposed on the aircraft distorted the enitire fusleage, including pushing the engine into the firewall. This distortion crushed the pilots legs. Control continuity was established for the rudder and the elevator . Control continuity was not established for the ailerons, due to a broken rod end. The throttle was found in the full increase position and the mixture was in the full rich position.
