What happened

The FAA’s narrative, in sentence case. The FAA writes it in capitals and removes private details.

On April 14, 2012 at 17:30, inspectors from this office arrived at Cedartown airport (4A4) to investigate an aircraft loss of control on departure from runway 27 at Cedartown. The accident occurred at 1800Z- 1400 LCL while departing on runway 27. The aircraft a Cessna 150 modified to a tail wheel configuration with 2 occupants experienced a loss of control at approximately the 1000 foot location and continued approximately 400 feet further with the left wing high and scraping the right wing tip on the runway in an arc to the left. There was indication of the right tire sliding due to maximum breaking by the PIC on the right only according to the PIC. As the aircraft exited the runway on to the grass the right main wheel dug into the gravel and broke away from the aircraft. The aircraft spun around and rested approximately 50 feet from the runway edge heading 350 degrees. A close examination of the aircraft confirmed substantial damage as follows. The propeller was bent rearward approximately 4 inches from the tip on each blade. The right main landing gear was separated from the aircraft and was located approximately 20 feet from the edge of the runway. The gravel between the wheel and tire along with the marking near the runway are consistent with a steep bank angle to the right before the gear tore away. The saddle structure was located near the separated gear. The right wing was bent up at a 30 degree angle just outboard of the lower wing strut. The fuselage and associated structure at the main gear attach point has evidence of compression and structural overload. The right horizontal stabilizer and elevator were observed bent in a similar manner to the wing. The aircraft was resting on the right wing. The aircraft total time at the time of the accident is 6718.6 tach and 527.9 hobbs. The last annual inspection was complied with at 6687 hours on 07/01/2011. CFR part 91.411 and 413 test and inspections were complied with in April of 2010 (next due 05/2012) before the arrival of the FAA iic the aircraft was relocated further back from the runway with the concurrence with the FAA iic and photographs documented the original locationthe interview with the pilot in command privacy data omitted revealed the following:-privacy data omitted holds an FAA ATP and flight instructor certificate and has 100 hours in this make and model aircraft and 15,000 hours total time. privacy data omitted Holds a valid first class medical certificate that was issued on 12/08/2011.-privacy data omitted has logged 200 hours in the last 90 days and 350 in the last year.-at the time of the accident the weather was VMC with winds out of the east south east at approximately 9 knots. Rwy 27 was in use by the parachute jump aircraft operating at 4A4. The winds however, would suggest the use of 090. -according to privacy data omitted, the following took place. "Aircraft was taxied for departure to runway 28 at K4A4 (Cedartown ga ). Take off roll was normal for 300 feet, holding forward stick tail was brought to the t-o attitude. A/c began to swerve to the left in spite of brake and control input. Swerve continued, a/c left runway and right main gear left aircraft. A/c skidded to stop. Occupants exited aircraft thru pilot side door. There were no injuries.-when the aircraft was lifted the landing gear area was examined. The damage noted on the right side was consistent with overload. -the aircraft ELT was not activated during the accident.-the last annual inspection was complied with on July 01, 2011 at 6687 hours.-the total time at the time of the accident is 6718.6 hours.

As the FAA filed it
