From the FAA’s Accident/Incident Data System (AIDS), through its Aviation Safety Information Portal, as reported. The NTSB investigates accidents separately, and its record of an accident is the fuller one. The aircraft is matched to the registry by N-number and event date, checked against the reported make and model; the registry’s history has gaps, so older matches can be wrong. Compare the reported make and model.

What happened

The FAA’s narrative, in sentence case. The FAA writes it in capitals and removes private details.

Investigated occurrence of unscheduled landing of this rotorcraft during a cross country CFI training session. Approximately 10:00 PST after departing hio, the "clutch" annunciator illuminated on the aircraft. The PIC, privacy data omitted, waited for 10 seconds as prescribed by the R-22 rfms. He then disabled the system by pulling the"clutch" circuit breaker. He located a suitable landing zone at the robert gray middle school athletic field and executed a non-eventful precautionary landing. The aircraft landed under power and no autorotation was exercised. After landing, the aircraft was secured and the Portland police department dispatched an officer to the scene. I arrived at approximately 10:45, photographed the aircraft, verified the clutch mechanism was inoperative, interviewed the crew members and released the aircraft for transport.
