From the FAA’s Accident/Incident Data System (AIDS), through its Aviation Safety Information Portal, as reported. The NTSB investigates accidents separately, and its record of an accident is the fuller one. The aircraft is matched to the registry by N-number and event date, checked against the reported make and model; the registry’s history has gaps, so older matches can be wrong. Compare the reported make and model.

What happened

The FAA’s narrative, in sentence case. The FAA writes it in capitals and removes private details.

According to the pilot, this lifeguard flight originated from west, Texas following a fertilizer plant explosion with multiple deaths and injuries. The pilot saw a bird about four feet from the helicopter canopy while traveling at 120 KTS. The bird hit and penetrated the windshield on the right side, about the level of the instrument panel. The bird struck the back of the instrument panel, denting the case, hit the pilot?s arm and right door frame and several areas inside the cabin, finally coming to rest at the patient?s feet.
