From the FAA’s Accident/Incident Data System (AIDS), through its Aviation Safety Information Portal, as reported. The NTSB investigates accidents separately, and its record of an accident is the fuller one. The aircraft is matched to the registry by N-number and event date, checked against the reported make and model; the registry’s history has gaps, so older matches can be wrong. Compare the reported make and model.

What happened

The FAA’s narrative, in sentence case. The FAA writes it in capitals and removes private details.

At the time of the incident, instruction was being given in the aircraft on a private grass strip. As the student pilot was performing the take-off roll, the aircraft began to drift slightly to the left of the runway centerline. He over corrected to the right and then over corrected to the left. The flight instructor took the controls to correct back to the right. During this process, the aircraft had accelerated to take off speed so the instructor rotated off of the ground several feet and the aircraft continued veering to the left. The aircraft contacted a small hill just off the left edge of the runway breaking off the left main wheel. The aircraft spun to the left, the propeller contacted the ground and the left wing tip. The aircraft slid up the hill for approximately 75 feet and came to rest. No injuries were reported. The aircraft did not sustain substantial damage.
